has anybody undertaken extending the cab on a twin cab? Im sourcing bits at the moment in readiness for a new extension

you'd have to stretch it under the cab on the straight section pushing the rearmost cab mounts back to pick up the new position. extended propshaft and services to suit the new wheelbase. I don't think the undercarriage work would be that onerous to be honest, its the cab itself and trim that will take up the time.The rear wheels are a bit in the way for a third pair of doors, but it could be done.
I very much doubt that the OP could achieve what they want without modifying the Chassis and extending it on top of that modifying the wheelbase to accommodate the changes. They are at least going to lengthen the vehicle by 6ft.An SVA isn't needed if it is just the body that is being modified on a non monocoque vehicle.
